In July 2025, our Area Agency on Aging partners shared their work at the national USAging Conference. Their presentation, Bridging the Gap: A Toolkit for Strengthening AAA Relationships with Healthcare Systems, highlighted innovative strategies for building stronger partnerships between healthcare systems and community-based aging services.

Area Agencies on Aging (AAAs) play a vital role in helping older adults, caregivers, and people with disabilities live with health, independence, and dignity in their homes and communities.

As part of the presentation, we featured our Primary Care Liaison role and the collaboratively developed Implementation Guide—a practical roadmap designed to help other AAAs across the country adopt and adapt this model within their own communities.
