Self-Management Plans for Patients, Family, and Caregivers

We partner with Aging and Disability Services (ADS) of Seattle-King County to create and disseminate Self-Management Plans for patients, family, and caregivers. There are over 25 plans available in multiple languages to help patients, family, and caregivers manage common chronic health conditions. The plans use traffic light flags to help decide when to seek care from a health professional (green = good health, yellow = warnings, red = emergencies).

ADS collaborates with community partners, including hospitals, Medicare QIOs, and various professional disciplines, to develop patient education materials for common chronic health conditions. The plans incorporate Age-Friendly Health Care’s 4Ms framework.

New Poster Presentation: Virtualizing our Area Agency on Aging Practicum

Congratulations to our three Primary Care Liaisons (Breanne Swanson, Phung Nguyen, and Felicia Sanchez) and their well-received poster, Virtual Area Agency on Aging Based Practicum for Health Professions Trainees at the American Society on Aging’s Annual Conference On Aging 2023 in Atlanta, GA. Our Area Agency on Aging (AAA) Practicum is one way we work towards bridging the clinic-community gap by teaching primary care trainees (geriatric fellows, family medicine residents, and advanced practice nursing students) about programs to address the social determinants of health needs of older adults and caregivers. During the pandemic, our practicum was re-invented virtually using asynchronous videos and Zoom debriefings. Our findings indicate that our virtual practicum experience is just as valuable as it was pre-pandemic and if anything, it’s more convenient for busy primary care trainees.

Center to Increase Health Workforce Geriatrics Expertise

A Northwest Geriatrics Workforce Enhancement Center is being established at the University of Washington to increase the expertise of primary-care providers, trainees and others in caring for older adults.
The ultimate goal is to improve healthcare for aging individuals and populations in the WWAMI (Washington, Wyoming, Alaska, Montana, and Idaho) region.

The UW Division of Gerontology and Geriatric Medicine received a three year, $2.5 million cooperative agreement award from the U.S. Department of Health and Human Services’ Health Resources and Services Administration for the new center.

The grant is part of the $37.5 million in funding to 44 organizations around the country announced by President Barack Obama and HHS Secretary Sylvia M. Burwell at the July 13 White House Conference on Aging.

Dr. Elizabeth Phelan, UW associate professor of medicine, will be the project director of the center. The co-director is Dr. Michael Vitiello, professor of psychiatry and behavioral sciences. Dr. Barbara B. Cochrane, professor of family and child nursing, will be the associate director.

The Division of Gerontology and Geriatric Medicine at the UW medical school houses the center. Other UW units contributing geriatrics practice expertise and leadership include the UW Schools of Nursing, Pharmacy and Social Work. Partner organizations include the Veterans Affairs Puget Sound Health Care System, Aging and Disability Services of Seattle/King County, Area Agency on Aging and Disabilities of Southwest Washington, and Qualis Health.
