Our Primary Care Liaisons, Felicia Sanchez (ALTC, Yakima, WA) and Breanne Swanson (AAADSW, Vancouver, WA) presented at a W4A or Washington Association of Area Agencies on Aging Conference (June 2024). They spoke about how to utilize the framework of healthcare partnerships to expand the  impact of Area Agency on Aging (AAA) programs like Care Transitions. Also, how it will be important to provide education to providers on upcoming changes that will impact the long term care landscape for people in the future (WA CARES).
