New Publication: Improving Age-Friendly Advance Care Planning in Primary Care: Outcomes from a Pacific NW Learning Collaborative

Helping Patients Share What Matters Most. Advance care planning (ACP) helps ensure patients’ voices are heard by encouraging conversations about their health goals and future care. When aligned with the Age-Friendly Health System’s “What Matters” approach, these conversations can be even more powerful.

That’s why our team brought together 6 primary care clinics from across the WWAMI region (WA, WY, AK, MT, ID) for a 9-month virtual learning collaborative. With expert guidance, training sessions, and support, providers boosted their confidence and skills in having these important discussions.

Results? Increased knowledge, more ACP conversations with patients and families, and greater documentation in the medical record in most practices. A meaningful step toward more personalized, age-friendly care!
